Elder ServicesWithin all our clinical programs, staff who are trained and experienced in working with elders provide services to those aged 60 and over. Services for EldersServices are provided at our licensed outpatient offices and in community settings, such as elderly housing complexes, nursing homes, assisted living facilities, and private homes. All services for older adults have a goal of improving health and assuring a quality of life in the community and/or the least restrictive environment that is safe and healthy. NRI Community Services also collaborates with the Visiting Nurse Service of Greater RI, the Department of Elderly Affairs, and other health and human services agencies to enhance the availability and accessibility of a full continuum of care for elders. Casework, Referral & Advocacy — CRAOur CRA program is certified by, and has a regional contract with the RI Department of Elderly Affairs (DEA). A multidisciplinary team provides comprehensive, home-based services for elders living in Coventry and Washington County who are at risk for abuse, self-neglect, or who are in need of services to "age in place" within their homes. ReferralsDEA, as well as family members, elder/social service providers or members of the community, make referrals to this program. CRA ServicesCRA clinical and nursing staff provide client centered geriatric assessments and case management, caregiver support, facilitated referrals to health and other social service agencies, advocacy services, and environmental (home) risk assessments. Staff from NRICS are also involved in elder care coordination, planning and system development activities through collaboration with several community elder care organizations. |
